Smoking Cessation Service De-commissioning Timetable


Information about the end of the Together 4 Health Lifestyle Services (including smoking cessation) in Staffordshire following decommissioning notice by Staffs County Council.

Please note: Stoke on Trent City Council services are not affected by this change.

Notice has been given by the county council that ALL Together 4 Health Lifestyle Services are terminating at the end of March 2017.  Please read all of this notice which has information about actions which pharmacies need to carry out as well as details of how the payments to pharmacies will be managed as the service winds down.  Every pharmacy should receive a visit from a member of the Together 4 Health team to explain the process and this information has been prepared to further explain the timetable.

Remove display material NOW.

Remove ALL display materials (posters, leaflets etc) which mention Together 4 Health OR Time to Quit

This is because the service is ending and no new clients can now enter the service, therefore no promotional leaflets about the service should be left on display.  Please remember that Time to Quit was the original name of the service which then changed to Together 4 Health so all display materials with either name on should be removed from your pharmacy now.

Any generic Stop Smoking posters (eg Department of Health etc) which do not mention Together 4 Health or Time to Quit can be left in place.

Signposting NEW clients asking about smoking, alcohol and weight management.

You should be aware that ALL stop smoking advisor services which were commissioned by the county council are being decommissioned.  You therefore cannot signpost the client to another pharmacy or any other Together 4 Health advisor service in Staffordshire because none of these advisors are able to accept new clients.  The service is closed to new clients.  This is non-negotiable.

Any client asking about smoking cessation, help with alcohol consumption or weight management should be given the following advice.

Clients already in the smoking cessation service

Any client who has already set a quit date will continue to be supported, more information is supplied throughout this update.  There is information about the supply of NRT against vouchers and also information about the advisor service.  Please read carefully as the Together 4 Health team are working with the LPC to ensure pharmacies do not lose out BUT you must meet the deadlines outlined here.

Together 4 Health Vouchers for NRT Supply: Voucher validity and Payment DEADLINES

While clients already in the system are still being supported the advisors will issue vouchers in the usual way and you are able to accept these and supply the products in the usual way.

The last date that vouchers will be issued is March 23rd 2017.  The last time the client is seen by their advisor, if they have achieved a 4 week quit and are still abstinent, they will be given enough vouchers to provide them with NRT to last until they reach the end of their 12 week programme.

This means that you may get some clients presenting multiple vouchers on or around March 23rd and all of these vouchers will be dated the same date (they should not be post-dated).

Supply the NRT as normal and enter the information on PharmOutcomes.

Last date for entry of voucher information and claims onto PharmOutcomes is April 6th 2017 and the last invoice will then be runTogether 4 Health are being decommissioned and late claims will NOT be paid as the service will no longer exist, therefore it is VITAL the community pharmacies enter this data before the cut-off date.

Together 4 Health Smoking Advisor Services: Client service provision and Payment DEADLINES

For pharmacies who have contracted with Together 4 Health to provide the full advisor service you should now have received a message via PharmOutcomes, from the LPC and also had a visit from Together 4 Health to explain the process.  To summarise what is happening:

You may have clients already in the system who have a Quit Date up to and including Feb 2nd 2017.  You should continue to support these clients and issue NRT in the usual way with some slight changes to the process as outlined below.

Quit Attempt with Quit Date up to and including January 4th 2017.

These clients should be able to complete their quit attempt and any who achieve a 12 week quit should be claimed in the usual way on PharmOutcomes.

Last date for entry of these claims onto PharmOutcomes is April 6th 2017 for inclusion in the last invoice runTogether 4 Health are being decommissioned and late claims will NOT be paid as the service will no longer exist, therefore it is VITAL the community pharmacies enter this data before the cut-off date.

Quit Attempt with Quit Date January 5th 2017 or later.

You may have a small number of these clients and they will not be able to complete their quit attempt before the service is terminated.  It would be impossible for them to have achieved a 12 week quit in this timescale and so special arrangements have been made for the pharmacy to claim a payment.  Together 4 Health have decided to deal with these clients as follows.

NRT for Quit Attempt with Quit Date January 5th 2017 or later:

The absolutely latest date that you should supply clients with NRT is March 31st 2017 so in the weeks leading up to the end of the service you will need to plan for the last supply for these clients.

At the last consultation in March AND where the client has achieved a 4 week quit AND is still abstinent you may provide NRT products to last them up until the end of their 12 week attempt.  This will be after the Together 4 Health Service ends so you will need to supply multiple batches of NRT at this last appointment.

The NRT should be claimed in the usual way on PharmOutcomes, if you are supplying more than 2 weeks worth of NRT you will need to enter each 2 week supply as a separate entry on PharmOutcomes.

Last date for entry of these claims onto PharmOutcomes is April 6th 2017 for inclusion in the last invoice runTogether 4 Health are being decommissioned and late claims will NOT be paid as the service will no longer exist, therefore it is VITAL the community pharmacies enter this data before the cut-off date.

12 week quit payment for Quit Attempt with Quit Date January 5th 2017 or later:

All 12 week quits and post-delivery quits for pregnant women have to be claimed by April 6th and these clients with quit dates of Jan 5th or later will not have been in the service long enough to achieve a 12 week quit.

A decision has been taken by Together 4 Health that when you see these clients for the last time in March AND where the client has achieved a 4 week quit AND is still abstinent your pharmacy will be paid a 12 week quit payment.

A new module is being written for PharmOutcomes which will collect a small amount of data about these clients and this will trigger the 12 week quit payment.  This module will be released to smoking cessation advisor pharmacies in March.  The new module will only accept information about clients with a quit date of Jan 5th or later.

Future pharmacy smoking cessation service?

Please be aware that no replacement service has been commissioned, there is nowhere to signpost clients to for free NRT.  The healthy hub number is to be given so people can get information about Staffordshire support for lifestyle/behaviour change and is the only place you should signpost to.

If the LPC becomes aware of an opportunity for pharmacies to be involved in any smoking cessation services going forward we will advise pharmacies as soon as information becomes available.
